Jack meets Kate and really likes her.
He invites her to prom and she stands him but - he doesn't know why until he finds out she's in the hospital, really sick. He visits for a while and goes home, only to be called later that she has passed away.
He wakes up the next day and realizes he's able to rewrite history.
So the rest of the book is him rewriting their history so that he can spend more time with her.
